Relocating or thinking about switching health care providers? Transferring to one of our offices is easy. We see patients aged 0 to 21 in our offices. To begin the transfer process, check out our medical record release form below.

It’s Easy to Transfer

Complete a medical record release form
If you have multiple children, we will need a form for each child
We'll fax the release to your previous doctor to transfer records
We'll contact you once all* prior records are received
*in order to provide the most comprehensive care to our new patients, we require 3 years of prior medical records and a current immunization record to establish care

Self pay patients

All patients not enrolled in a health plan will be required to pay at check in. These fees will cover 100% of the visit; no other amount will be billed.
$100 for sick appointments
$140 for well child checks
$60 for sports physical
* some health plans do not cover the cost of sport physicals or well child
appointments. For these plans the self-pay rate will apply.
