Verify insurance information
First time users can easily establish their healow accounts through the patient portal or Healow app. Simply log in using provided credentials or by verifying key demographic information.
Forms
All forms can be submitted by fax to 803-520-5972, or you can bring them with you to your visit.
Immunization records can be requested by sending a general message through the healow Patient Portal or by calling our office. While we will do our best to get your record prepared as quickly as possible, please be aware that it may take up to 5 business days to prepare since our nurses need to review the records and we have a high volume of requests during certain times of the year.
