HOW TO JOIN OUR PRACTICE

Getting started with SouthernMED is easy.

Getting started with SouthernMED Pediatrics is designed to be straightforward for busy families. We guide families through registration, help arrange your child’s first appointment, and assist with transferring medical records so the transition to their new pediatric practice goes smoothly.

    Complete Patient Registration

    Begin by completing our secure online registration form for each child. This provides the information we need to create your child’s medical record. After reviewing your submission, our team will contact you with the next steps.

    Transfer Medical Records

    If your child previously received care from another pediatrician, we’ll help coordinate the transfer of their medical records. This gives your SouthernMED provider access to important health information, including immunization history, previous visits, and current care needs.

  • 3.

    Schedule Your First Visit

    Once our team has reviewed and accepted your child’s registration and you’ve requested their medical records, you can schedule the first appointment online, by text, or over the phone. Before the visit, Phreesia will send you a secure pre-check-in form to complete in advance.

Your child’s first visit at SouthernMED Pediatrics gives our team an opportunity to establish care and learn about their medical history. Your provider will review available records and immunizations, discuss current health needs, complete a physical exam, and answer your questions. Before you leave, we’ll explain any recommended screenings, follow-up care, or future well visits.

Availability may vary by location and provider, but we can typically see your child within one to two weeks.     New patients begin by completing our online registration form before scheduling an appointment. Once our team reviews and accepts your registration, you’ll receive confirmation and can schedule your child’s first visit online. This review helps us gather the necessary information and match your child with the appropriate appointment.

  • What if my child needs care before our first scheduled visit?

    If your child needs care before their first appointment, please call or text our office. Our team will help determine the appropriate next step and make every effort to address urgent concerns. Depending on availability, we may be able to arrange a same-day visit at one of our SouthernMED locations.

  • How do I transfer my child’s records from another pediatrician?

    After you complete registration, our team can help coordinate the transfer of your child’s medical records. To begin, contact your child’s previous provider and request that the records be sent to SouthernMED Pediatrics. Having this information available before the first appointment helps your new provider understand your child’s health history and maintain continuity of care.
