Medical Records Requests
To request a complete copy of your child’s medical records, please submit the Medical Records Release form linked below. A separate form is required for each child. Patients age 18 or older must complete and submit their own request.
There may be a fee for processing of records as defined in SC State Law Section 44-7.130.

Requesting Vaccination Records?
A Medical Records Release form is only needed when requesting a complete copy of your child’s medical records. To request vaccination records, text “SHOT” to 803-866-7337 or call our office. Please allow up to five business days for processing, as our nursing team must review each request. Requests may take longer during periods of high demand.
